Clicking on the links marked (Online) will open an interactive version of the registration packet that you can fill out online -- simply type your information into the form and print out the completed copy to bring with you to your first office visit. Financial Hardship Program

Rosalind Franklin University Health System's Financial Hardship Program (FHP) is available for eligible individuals who can demonstrate a genuine need for discounted medical care. If your FHP application is approved, you will receive a discount on the amount you pay for medical services from the Health System for a set period of time, usually 12 months.

In order for your application to be considered, you must complete all required paperwork and submit all required financial documentation. Instructions for filling out and submitting your application can be found on the first page of the application.

To obtain a copy of the application, click the link below and print out the application. Once you have completed filling out your application, submit all your materials to the address listed on the first page of the application.
